The Saint Joseph's Center for Diabetes Care
The Saint Joseph's Center for Diabetes Care designs complete diabetes management plans for its patients. Based on a multidisciplinary approach to care, the center emphasizes early detection and intervention and focuses on ways to prevent or delay the complications of diabetes.
Tailored for each patient by a team of health-care specialists, the program emphasizes self-management training and care including meal planning, exercise, blood-glucose testing, medications - and more. This self-care element of disease management is vital for the control of diabetes.
Studies show that the early diagnosis and proper treatment of diabetes can dramatically reduce, or even prevent, diabetes complications. Saint Joseph's ultimate goal for diabetes management is to keep blood-glucose levels as close to normal as safely possible. Controlling glucose levels through improved disease management will ultimately lead to a better quality of life for people with diabetes.
